Terms and Conditions of Use

Carle is pleased to offer you as a Carle or other community connect patient, patient information via a web-based version of our computer systems.

Your patient portal enrollment is contingent on verifications of your identity. User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a patient portal user, your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis, and 2) Keeping your login ID and password confidential.

We think it is important for you to know how we handle information we communicate via the internet. This Terms & Condition statement outlines our practices and our sensitivity to the privacy and security of your medical information.

Inappropriate use/termination of access

Your patient portal access is granted for the purpose of furthering your medical care. Use of this portal unrelated to this purpose may result in the suspension or termination of your access privileges. Improper uses include, but are not limited to, the use of inappropriate, threatening or abusive language, requesting appointment times that are not needed or are frequently cancelled, and any other use that Carle determines in its sole discretion constitutes a disruption to Carle and community connect partner operations. Carle reserves the right, in its sole discretion, to terminate a user’s access to all or part of the patient portal website, with or without notice.

Confidentiality and security

The privacy of your health information is extremely important to us.

Carle will maintain your health information in strict confidence and will not disclose it to any unaffiliated third party unless you authorize that person to receive your health information or it is permitted to be disclosed by law. Please review your entity’s privacy notice for an explanation of how, when, and why the entity uses and discloses your health information. All electronic information received within your patient portal that contain health information are subject to all state and federal laws governing the confidentiality and security of medical records.

All communication between you and your care providers occur over a secure connection. All of the health information available to you is accessed within your patient portal account and is protected and securely maintained by Carle. Although your patient portal is configured to be secure from unauthorized access, Carle and community connect partners, are not responsible for:

  • Absolute security of all electronic communication transmissions between the patient and Carle and community connect partners;
  • Unauthorized disclosure resulting from a user not logging out of an active session;
  • Unauthorized disclosure resulting from a lost, stolen, or shared login name and/or password;
  • Unauthorized disclosure resulting from information printed from the patient portal by the user;
  • Unauthorized disclosure resulting from personal computer setting or installed software products that may compromise information security; or
  • Similar events beyond the control of Carle and community connect partners.

Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We shall identify the records released and note the time and date of access each time a patient accesses their patient portal. We have taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use.

For other general information viewing, the patient portal must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (Netscape or Internet Explore versions 5.0 or greater). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available (128-bit RC4).

You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Carle by viewing the digital certificate. This certificate verifies the connection between the Carle server’s public key and the server’s identification.

E-mail Privacy

Patients who are users of the patient portal should be aware that they will be notified via e-mail when there is new medical information to view. This means that any person with access to a patient’s e-mail will be able to see this notification. This could include the patient’s spouse, employer or anyone else that can access a patient’s email account. Although no private medical information will be sent, the notification that new medical information is available by accessing the patient portal may be information that a patient would not want others to know. Thus, you should take this into account when providing an e-mail address.

Response to Electronic Communication

You should never use your patient portal for urgent matters. For all urgent medical matters, contact your physician’s office by phone, and/or go to the emergency department of a local hospital, or dial 911.

Your reliance on the information provided in the patient portal is not a replacement for proper medical attention. The information displayed in the patient portal may not be the complete medical record. Therefore, you must contact Carle or its community connect partners for official and complete copies of your medical records.

Property Rights

The content and materials available on the patient portal website are protected by copyrights, trademarks, patents, trade secrets, or other proprietary rights and laws. You may download, use and copy materials from the patient portal website for your personal, noncommercial use only, provided all copies include our copyright information and logo, and the information is not altered in any way. Materials cannot be copied and redistributed without the express written permission of Carle and/or community connect partners. Except as authorized in this paragraph, you are not being granted a license under any copyright, trademark, patent, or other intellectual property right in the material or the products, services, processes or technology described in this website. All such rights are retained by Carle, its affiliates and subsidiaries, and/or any third-party owner of such rights including Carle’s community connect partners. You may not create framed links to the patient portal website without the express written permission from Carle.

Disclaimer of Warranties and Limitations of Liability

Carle and community connect partners make no representations or warranties about the suitability of the patient portal website materials for any purpose. The materials on the patient portal website are provided: WITHOUT WARRANTY OF ANY KIND, EITHER EXPRESSED OR IMPLIED, INCLUDING, BUT NOT LIMITED TO, THE IMPLIED WARRENTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE OR NONINFRINGEMENT. Neither Carle nor other community connect partners warrant the accuracy or completeness of the information, text, graphics, links and other items contained within these materials.


Carle may make any changes to these materials, or the products and services described within these materials, at any time without notice. Carle makes no representation or commitment that it will update the information contained herein.

Legal Compliance

Users are prohibited from posting or transmitting any unlawful, threatening, libelous, defamatory, obscene, scandalous, inflammatory, pornographic, or profane material on the patient portal website. If something a user posted or transmitted results in or encourages conduct that is considered a criminal offense, civil liability, or otherwise violates any law, Carle will fully cooperate with law enforcement authorities or court order requesting or directing Carle to disclose the identity of anyone posting such information or materials. Carle will direct community direct partners to comply in the same manner.

Choice of Law and Forum

The terms and conditions set forth above shall be governed by and construed in accordance with the laws of the State of Illinois. All users expressly agree that the exclusive jurisdiction of any claim or action arising out of or relating to these terms and conditions or any use of this website shall be filed only in the state or federal courts located in the State of Illinois. Further, all users agree and submit to the exercise of personal jurisdiction of such courts for the purpose of litigating any such claim or action.

Severability and Integration

Unless otherwise specified herein, the terms and conditions stated above constitute the entire agreement between the user and Carle with respect to this website and supersedes all prior or contemporaneous communications and proposals (whether oral, written, or electronic) between the user and Carle with respect to this website. If any part of the terms and conditions is held invalid or unenforceable, that portion shall be construed in a manner consistent with applicable law to reflect, as nearly as possible, the original intentions of the parties, and the remaining portions shall remain in full force and effect.
